Consultations: New client office visit is $250, established client office visit is $100/hour. SHAPE ReClaimed is designed as a package program.
Scheduling: Telehealth consultations are available via Zoom or phone.
Cancelations: If you need to change your appointment, I ask that you do so as far in advance as possible. You will be charged a cancelation fee of $50 if I do not receive at least 48 hours advance notice. I appreciate your cooperation, as appointment time is limited and many, like yourself, are anxious for these services.
Payment: Is due when the invoice is received. Payments are made by clicking the “pay invoice” button to pay securely online from your bank account.
Insurance: We do not bill insurance, although you might consider submitting a receipt to your insurance provider directly.
