Consultations: Fees are $80/hour. Phone consultations are available also.

 

Scheduling: Appointments are available on Monday, Tuesday, and Thursdays.

 

Cancellation Policy:  If you need to change your appointment, I ask that you do so as far in advance as possible by telephone.  You will be charged a cancellation fee if I do not receive at least 24 hours advance notice.  I appreciate your cooperation, as appointment time is limited and many, like yourself, are anxious for these services.

 

Payment of Services:  Some insurance companies are now recognizing and covering fees for nutritional services and preventative health.  To better serve your healthcare needs, I am now offering insurance billing receipts with specific coding and documentation which you may submit to your insurance company for possible reimbursement. 

 

Payment via cash, check, or credit card is due when services are rendered.

 

Returns:  Unopened products purchased within the previous 30 days may be returned for credit towards product or services.
